I think a 2all of DDP Campaign edition, or Image Fight via the penalty area are considered the two hardest clears, at least according to that list that has been posted on here now and then from Japan.

Hardest shmup I've ever personally played is either Dimahoo, Battle Garegga or Gunbird II.

But that's just my opinion.

This has only been cleared once ever, and even in this clear he has to use absolutely every resource available and do insane dodges through patterns that should kill any mortal player, so it is worthy of the title 'hardest shmup ever'.

EDIT 2: I thought of a good third place for my list: Muchi Muchi Pork Manpuku 2-ALL!
Manpuku combines a ridiculous torrent of suicide bullets from almost everything in the game (including suicide circles, splurging 'lasers' of bullets, etc) with the over-the-top patterns of loop 2 (especially miniboss and post-miniboss sections with such dense bullets as to feel almost impossible to safely clear!). Your only saving grace is that you can get score extends indefinitely, but no no one has ever cleared this.

The only reason why I would not put it higher than third despite having never been cleared ever is that this gameplay mode gets very little attention compared to Futari Ultra and DDP DoJ DL, so it's possible that it would have easier strats/a clear by now if it were more popular.

I find Recca and Ketsui reasonably hard, at least at the beginning. I'm sure there are many levels of nastiness beyond them.

Every time I'm linked to that Ultra mode video I end up watching it. And every time, I'm baffled.

BGR-44 was accused of cheating some time ago. There are probably reasonable doubts concerning his potential Death Label clear.

I also didn't know that SWY has cleared it. Is there any information about this out there?

...That I've played? Probably Gunbird 2. Especially 2-ALL. videos of that clear are intense as fuck.
Recca is also pretty damn hard, in my experience. Mostly when you accidentally get the wrong powerup. Might be a little easier if you lower the ship speed, but IDK really. It probably makes some things harder, too. I doubt it's actually as hard as a GB2 2-ALL.
