Yes, came in new kits (afaik) bought by two reputable collectors on completely different sides of the world. B1 always says Kit to me (although I can't guarantee the exact size, it's 2nd hand info) and folded never says matsuri, hence my confusion. Purchased from Fujita in 2009 and it came with the birdcage poster.
Kit two:
Purchased from what I assume is a group buy gone bad in 2011. The seller had 6 kits. Each one came with a folded version of the poster in question. The kits were described as NOS.

I bought one of the last Fujita Deathsmiles kits (I think the very last one to be honest), rolled B1 birdcage poster came with it. That B2 poster in the pic definitely came out after, but there is nothing stopping distributors from "making their own kits" and putting them in to try and enhance sales (especially if the posters were not selling alone). In short, distributors put whatever the they wanted into kits, flyers from other games. Trying to document an authoratative list is impossible.

Just a small update: Found that Guwange's original A4 instruction sheet was printed in blue ink. Can someone confirm if the reprint was plain black on white? I recall that it was, but need clarification.

They are very different .
Galuda 2 one has a hole in the middle to insert the base of the promo disc spring, the Ketsui one has a small plastic hook fixed in the middle to attach the flyer baggie .
