Easy nomination - Capcom's 1942 - good god, that music drives me around the bend, and what makes it worse is the annoying whistle blowing sounds.
Then there's the weedy sound f/x.
Pathetic really considering the game itself is a lot of fun.

haha- I was going to say Xevious too. Isn't it like a 3-5 second music loop?
One other that comes to mind is Raiden Fighters (only 1 or 2 of them). I enjoy alot of the music- sound effects are 'eh' (I really really love the games- don't get me wrong!) but some of the stages are obnoxious.

I love Mushihime-sama's music, but the death scream grates on me like few sound effects can.

RFA music as a whole generally sucks. There is a few good tracks though.

The music to Vulgus. It's not grating at all [like 1942], but there's something about it that is annoying as hell.

Um, everything in Giga Wing that isn't the title screen theme, the stage 6 theme, the ending theme, the reflect barrier noise, and "OTAY." Yeah, everything else audible in the game is horrible.

The announcer for the Self Destruct, who is probably the same kid who made it. Really fun and addicting game for what it is, and given it's an indie effort wouldn't expect any budget at all saying a sentence worth of words

OTAY and COME ON! are pure gold. Probably my favorite shmup voice clips besides the DonPachi announcer. I'd personally say Radiant Silvergun's music is my least favorite shmup sound. I first played the game in an emulator, and for the longest time I thought the sound was being emulated incorrectly, because the music sounded like ass. Then I played the game on a Saturn, and found out that that's just how the game sounds.

The BGM tunes on Data East's Darwin 4078 PCB circa 1987 is subpar. Even the BGM tunes found on Capcom's 1986 Hyper Dyne Sidearms sounds much better despite having been released a year earlier + considering the fact that both arcade shmup PCBs are mono output to begin with. Could it be that Capcom went with better chips for it's BGM tunes to begin with?

The Xevious music is so repetitive you can kind of tune it out, but listening to 1942 is like having someone piss in your ear.

I've always found the music in Team 17's game X2 really annoying- i've never been able to play it with music yet. Always switch it off.

No mention of Omega Fighter yet? The Saber weapon (the one that loses range when powered up) has such an annoying sound I avoid using it.

B-Wings, Xevious a close second.
Seriously to call B-Wings' BGM music is a stretch. It's the most migrain inducing, repetitive and annoying sound in any game in my opinion.

Irori from ESP RA.DE. That voice is so grating that t almost (just almost) turned me off from playing the game. Shit, every line that comes out of her mouth is singularly annoying.
