AFAIR, as far as aesthetic differences, the Megadrive version has a few more layers of parallax. Music is different too, at least some of it (I prefer the PCE version's music).

I thought I would really dig Twinkle Tale but lost interest in it really quickly. Probably because I purchased it at the same time as Undeadline which took me almost a week of obsessive play before I could finally beat.
Second choice would be Whip Rush but that's purely based off nostalgia rather than game quality.

As a kid i only ever played 2 shmups on my MD despite it been my main gaming system.
Gynoug which i rented a couple of times and really enjoyed it despite never getting past the "Penis man" boss.
Xenon 2 which i believe i owned a copy for a while, I was looking forward to trying it out since I was a fan of Bitmap Bros. I was
very disapointed as the sound track was the only good thing about it. Since i was never a fan of shmups when i was a kid and i had only played a few shmups and Xenon 2 been the only console vert i had ever played, I blame the fact the game was no fun to play as one of the reasons i never got into the genre. I was more of a fan of top down run 'n' gunners.
ATM i havnt played Musha, the Thunder Force series or Twinkle Tail
So i my vote goes for Gynoug but ill try out those 3 games and may comeback with a different answer in a day or two.

I favor the Turbo Grafx 16 version over the Genesis version. I purchased my Turbo Grafx 16 Aero Blasters back in 1992 at Toys R Us for a hefty price of $59.99 plus 4% sales tax. I was quite pleased with my purchase and got my worth out of that hucard.
There is one thing though the Genesis version of Air Buster offered that Turbo Grafx 16 Aero Blasters didn't…

Hell yeah, I remember the Toys R Us in Mt Clemins use to have killer deals on TG16 games! I remember buying Aero Blasters and Raiden For 19.99 and Side arms, Psychosis, and Cyborcore for 9.99! That was right around the time everyone was going crazy over the Snes and all Turbo games were marked waaayy down. Although I loved Aero Blasters, I think I like Air Buster a little better. Yeah, it's a case of dusting off the rose-tinted contacts whenever I play those old MD games. My introduction to the genre though, so those little style-over-substance titles will always hold a special place. Penis man boss...ahh.

I was saving up for an Amiga 500 when I saw an ad in my local paper: ForSale PCEngine,3 games,PALconverter,£90. I'd already seen screenshots on the back of a mate's C64 mag of R-type and Galaga '88 (I think), and knew milkybar engines generally went for nearer £150-200, so I jumped at the chance. The old dear even haggled the seller down to £80.
The following year I traded it for my first JPN MD. Happy days.

TFIV is pretty good, and epic. TFIII is more memorisizer-y and has crappy bosses that die in about 0.2 nanoseconds.
V-V/Grind Stormer is alright, but the stage 2 boss makes me want to stab whoever invented right-angle lasers with a right-angle knife.
Only reason I'd bother with Zero Wing is for the opening of doom.

It has to be Slap Fight. If not for that game I wouldn't have a MegaDrive (NTSC-J) and wouldn't have partially rebuilt my former Genesis collection.
The Genesis is a terrific console and there is lots of quality, cheap games to be found. Slap Fight isn't cheap but it is fun and obtaining that game has led to many other great games, such as Fire Shark, Pulseman, & Undeadline.

Empire of Steel - surely not the best or the greatest shmup on this
system ( thinking of Thunderforce III, IV, Gynoug, Musha Aleste .... ) ,
but a game that provides fun and has it´own graphical style -
reminds me and others on the books of Jules Verne.
Trouble shooter I and II : nice fun-shmups.
Hellfire: hori-shmup from Toaplan: graphics could have been better,
but interesting weapons.
Phelios : from Namco, the closest thing to Dragon Spirit or Dragon Saber
you find on the Mega-Drive/ Genesis. Can´t compete with the best,
but at least for me it provies fun . And who wouldn´t like it to fight
the monsters of the greek mythology.

Yup have to agree Steel Empire and Phelios are very underated. Vapor Trail would be right up there as well, but a shame that you can't loop it

The Data East released Vapor Trail arcade PCB does have the 2 loops to clear before the ending credits appear. All about memorization. Turn rensha mode on and only grab the twin daggers and you'll beat it eventually if you keep plugging away at it.
