Aaah, I had this confused with Aero Fighters 2. The beam doesn't kill you in the first loop of Aero Fighters 2, but it does in the second. Ikaruga : over 50 hours of play, and still no 1CC.
My last attempt ended with me loosing my last ship on one of the last bullet of that bloody diamond. Not even one from the growing circles that I all dodged successfully, but one from the few remaining spinning rays.
Timer WAS at 0 and game over appeared on screen : "we'll meet someday soon" <--- that soon has not come up yet.

Happened to me in a game of Gemini Wing. The final boss and I killed each other with our final shots. The worst part about it? I died what was supposed to be my final life and got a Game Over. However, the game went right to its endgame bonus tally and staff roll. The staff roll normally shows the ships in its cinematic, one or both depending on how many players finish the game. Running into a bullet after a boss has been killed. Seems to be happening a lot to me in Raiden Fighters Aces.

Being fully powered up in Blazing Lazers, and having a bullet hit the front of my ship instead of the shield and instantly sending me to pea-shooters.
Running into DESTOYABLE missiles in the first boss of Bakraid. From the side. With tickle laser at full power.
Killed by a yellow bullet on a yellow background. Thank you Darius II.
Killed by flicker. Can't see myself or the bullet that kills me on PCE R-Type.
Bird-bombing in Garegga using Flying Baron, and running into a bullet that my missiles didn't take care of. Oh, and killed by debris from bosses because its the same color as the bullet hiding behind it.
Switching polarities in Ikaruga to avoid those black bullets. While being pushed by a white laser.

In the 5.1 boss of ESPGALUDA, if you go to Kakusei overmode on the final pattern, some bullets "disappear" (on the PCB, not the PS2), yet they can still kill you.

Bullet flicker is the absolute bottom of the bad shooter design barrel. R-Type DX has one boss (II's fourth) turn into a matter of luck, basically. Even though it's on Gameboy Color, that really sucked for an otherwise nice conversion.
