Their is a flashing green light above slot 1 disk drive.
Does anyone know what this error means and what may have caused this?

Failure of soft power off can have many reasons. SRAM reset may fix the issue, but can also be early signs of hardware problems. My own XVI was running perfectly 2 weeks ago, but now it's lost video output. Now waiting for new inverter chips to arrive to fix it lol.
For grounding, just attach wire to the screw terminals and then connect to a ground. (Japan only uses 2 prong ac outlets and has wall screw jacks for ground cables).

There are lots of things that can cause failure of soft power off. If you have any modern upgrades (scsi sd, usb, etc), remove them all and try again. If soft power fails, check psu. If psu is good, then you got pcb issues.
Love my XVI, but these things are Ferarris. Work beautiful one day, then random failure the next.
