Cat Soup. Beautifully surreal, but also more coherent than people usually give it credit for on first viewing. I could watch this over and over again (it's only ~30 minutes) and never get tired of it.

Jan Svankmajer's oeuvre is highly worth investigating. His film adaptations of Alice in Wonderland, Faust and Otesaanek are all memorable, stunningly rendered trips in roughly escalating levels of unsettling.

Kill List (2011) is a perennial favourite of mine. Satisfyingly absolute self-destruction job. ^__^ Crime/Horror rabbit-hole hell dive. Our working-class hitman smashes pedos to bloody gristle and himself to subatomic dust.

Super (2010) is practically the same story in reverse. Self-affirmation via face-shattering DIY violence. Good fun. Affected kookiness falls a bit flat but Frank a legit crazy motherfucker.

maybe shane carruth's upstream color and primer?
kaufman is great (you listed synecdoche) but my personal favorite is being john malkovich if you haven't seen it.
not so "fucked up" but very stimulating.. gondry's l'ecume des jours (mood indigo) and the science of sleep.
jenuet's delicatessen

I love you. highly anticipating insects but I found out it's not being released till 2018... and I thought it was 2015. his shorts are definitely worth checking out too - food being my favorite.

I like Primer quite a bit, but didn't list it because I wasn't sure how "twisted crazy movies with surreal stuff" it counted as. Seeing what others have listed, though, I think it counts. I haven't gotten the chance to watch Upstream Color yet, though - how does that compare to Primer?

Oh, and I forgot to mention Masaaki Yuasa's Mind Game. Quite an interesting film (as expected of Yuasa) and definitely surreal.

The human centipede is f'd up.

A bit of catch 22 subject line if I ever saw one.

The movie that stayed with me the most is called Martyr's. Its a very morbid type of movie but it does keep your interest. Its very violent to the point you want to turn away from the screen on occasion. But the ending goes 50% to justifying the violence and the ending makes you think about stuff even days after you've seen it.
This industry has become 2 dimensional as it transcended into a 3D world.

primer is better, but it's hard to compare the two since they're vastly different. upstream color is a bit more straightforward/less convoluted/easier to follow, and even though it feels more "artistic" with long/drawn out scenic shots...the content is less intriguing. Still worth the watch though. I actually got to see it early 2013 as a double feature with Primer and a Q&A w/ Shane at the music box theater in Chicago which made the experience a bit more enjoyable. someone in the crowd asked him, "what's the movie actually about" and everyone boo'd him (lol). the buddy I went with had no experience with carruth films and he absolutely hated it, but he also dozed in and out of primer.

I really don't like this movie at all, but ExistenZ is definitely a fucked up movie. It's definitely changed my views on how far VR technology should and should not go.

A couple months ago I saw a movie just like The Prestige and has a similar theme whose name escapes me at the moment.

Now You See Me. Another movie like The Prestige, except this one might or might not even be worth seeing. Morgan Freeman is in it, so there's that.

Shutter Island is an excellent fucked up film. I'd rather not talk about it because I'm not sure what to mention without ruining it.

Not exactly fucked up but 2001 A Space Odyssey had a sequel to it, 2010 (forgot the subtitle). It's a shame they never finished the trilogy and made the third book into a movie as well.

Anything made by Tim Burton

There might be more that I've seen, but I can't think of any at the moment.

More Jodorowsky:
El Topo
Holy Mountain

Pretty much anything by Suzuki Seijun (though most of it's funny), start with Branded to Kill.

Woman in the Dunes and The Face of Another by Teshigahara Hiroshi might be worth checking out.

Yes, Blue Velvet is an oddity but then Lynch has made some interesting films over the years.

I would say that Troma's output would qualify for me. Poultrygheist: Night of the Chicken Dead presses most of the wrong buttons and it is pretty funny.

The first Exterminator had some 'wrong' scenes involving soldering-irons, young guys, pimp establishments and vasoline. The second part is simply hilarious and I think it is better.

It is a bit hard to track down (got the danish release) but Savage Streets with Linda Blair is also one to host the 'wrong' scenes.

Some good titles here, I'm adding the following;

The Eternal Evil Of Asia
Gozu
Come and See
Flesh+Blood
Fellini Satyricon

And many I forgot about, but honestly by a wide margin the best fucked up and hallucinatory stuff I've seen on film...was on short film.
Short films is another universe, superior in many ways to commercial cinema.
