Apparently it's part of the 500 Yen game pack (2015-03-05) for the free Yakuza Zero app for PlayStation Vita (2015-02-26; siliconera). The game goes back to the 2007 Windows game Beluga Mk.II by T. Matsushima. Seems that within a year that game appeared as Beluga 6001 on homebrew cartridges for the actual NEC PC-6001.

Oh, so THAT's what Beluga was all about. Spotted a post for that a while back on Shou's Tumblr page thinking it was some Sega Megadrive proto or some weird shit. Didn't know it was for the PC-Engine.
Apparently the translation "suicide aircraft carrier Beluga" is appropriate as you fight just to carve a path for the carrier aircraft to nuke the stage Kamikaze style.
