The only thing that comes to my mind is the older Castlevania games. What else is out there?

I would say Simons Quest, especially in the castles. That weird music and the corpses hanging down from the ceiling always send shivers down my spine.

Beyond the Forbidden Forest was probably the first game to creep me out, though that's not really 2D I suppose.

Not really "creepy" per se, but Capcom's Demon's Crest (SNES) has an excellent gloomy atmosphere.

Not sure if it counts, but it was an old adventure game called Dark Seed. The intro movie consists of a guy having his forehead ripped open and an alien seed implanted in his brain. I was only 8 years old at the time and it scared the hell out of me.

I've only watched youtube videos of Clock Tower but it looks really scary too.

Oh yeah! Splatterhouse III. I knew I was forgetting something. Its monster designs are grotesque (before you smash their faces in and get juices running), the time-limited family rescue system is pretty nerve-wracking, the cutscenes are brutal, and the music (particularly in those cutscenes) is chilling. It used to creep me out as a kid, still impressively macabre even now. The first two are pretty messed-up as well, but III feels less campy and more downright morbid.

Play the JP version if you're concerned about replay. The US one turns the crowd-control spin kick into a catch-all smartbomb that renders all other moves obsolete (and looks dumb, too).

Yeah, I recall that one. It was an HR Geiger game that looked like a Sierra On-Line adventure.

Clock Tower 1 and 2 are both pretty scary until you figure out how to survive. Once you've got hiding spots/weapons down, it's too easy to escape from scissorman and all the tension kind of disappears.

Clumsy as fuck. Mouse cursor + d-pad to move cursor = fail.
If you could use a mouse with this game all would be fine.

Definitely +1 to Super Metroid. Probably my all-time favorite game. Of all time.

And yeah, that sounds like Abuse. Never really creeped me out, but it's up there.

I'm gonna have to check out Dark Seed.

Super Metroid probably isn't anywhere near Clock Tower or whatnot, but when it came out i still was really young (i'm 21 now) and therefore easily creeped out.

When you're young a lot of stuff looks scary to you. Fighting the Cyberdemon as a kid was terrifying.

Aside from the "you're completely alone on this planet, good luck" feeling that it conveyed so well, every section was just well designed. I really can't find any faults with it. And it's kept some of the creep factor after all these years.

Another vote for Splatter House 3! The series as a whole is creepy and grotesque too. Yeah, even the Chibi one.

Cave Story. There's a lot of obscure detective work and exploration like Super Metroid and despite the numerous characters, still feels desolate.

Friday the 13th. I freaked out whenever Jason appeared all of a sudden. Especially when I had the rock.

And of course, Super Metroid like others have mentioned.
