Espgaluda Question

I recently got back into playing Espgaluda and I have a quick question.

I notice that on Stage 3, as I approach the boss, I sometimes get a heart shaped icon with an "L" on it; it is a free life. I cannot consistently get this, I maybe get it 2 out of 5 games or so. What triggers this free life?

Any help is much appreciated.

Also, this is a really dumb question, how many stages is the 1st loop of Espgaluda? I can get to stage 5 on one credit pretty consistently, but I am not sure if 5 is the last stage of loop 1.

Go into kakusei mode, start firing at the circular thing between the 4 cannons at the end of the stage. The extend item is hidden in there.

There's a very short bit after stage 5 (pretty much stage 5-2), then the last boss.

Also, there is no loop though if you're playing the PS2 port you can try your hand at Arrange mode.
