A Death Smiles conversion would be great, especially considering the hori orientation. The game is pretty popular and new, and even by its setting alone (little gothic girls) it would sell even more than Ketsui DL. Stage action would be a lot easier here, too, as I found Extra mode really unpleasant to play with all these enemies in crammed hori.
And yeah, seeing that Arika won´t do any Cave console ports anytime soon they might as well do some more of these handheld games.

They would be daft to not make more of these games, even from a business standpoint. And I agree - music and art assets are already there. I mean, Mihara did this game completely on his own, right? That sounds like a great manpower to profit ratio to me! And who knows, Cave shmups coming to the DS might even get more people to get interested in the arcade games.
(It might, however, still be possible that Ketsui DL only sold so well because of that DVD, but I can´t really believe that... I think it has more to do with japanese people loving portable games.)
