A couple of personal favorites:
Gundemonium Recollection -- Awesome doujin game, very fast movement all over the place. The only place where tap dodging is really applicable are a couple of patterns on the final boss! Similarly, everything by Murasame is very good; I highly recommend Hitogata Happa as well.
Mushihimesama Futari Original -- The other modes apply too, but would have lots more micrododging. This one will have you moving all over the screen for the duration of the game. Crank that rank up!
Yakouga 4 -- If you can get past the fact that this is sort of a hentai shmup, it's an awesome shooter with a ridiculously fast pace. Micrododging is completely irrelevant here, and the patterns are absurd, fast, and all over the place. Play on the harder difficulties.
RECCA -- No seriously, why hasn't this been mentioned yet? Greatest game ever made.
Already mentioned: Gunbird 2, Dragon Blaze.
EDIT: Oh, and DIMAHOO. Play as Miyamoto.

I'm probably not the best judge of this, because I've only made it through the first 2 stages, but I'd say that my movement in Recca tends to be pretty conservative when I succeed at it. It's more like: get search subweapon, power up normal weapons, stay against the bottom of the screen (because almost nothing with threaten you from that part of the screen). Occasionally it makes sense to dodge the bullets (usually macro), but absorbing them is often less risky. I love that charge bomb though. Oh good, I'm glad I was on the right track there. Definately I bet a lot of games with timed item chaining and fast movement speeds end up like this, btw. That seems to be a big part of Dangun Feveron's full screen movement stuff from what I could tell from watching a gameplay clip. I've always wanted to get into Hitogatta Happa, but the mechanics in that game terrify me more than the H.R Giger abominations in Yakouga, ha ha ha.
So far I think Batrider is the best example mentioned in this thread. The player hitbox and patterns seem tailored to keep things frantic.

Dangun was already mentionned several times, so I'll go with Parsec47 instead. In Extreme mode at least, there are many enemy patterns that force you to quickly move from one side of the screen to the other. Parsec47 also has the discomen system and it is related to the extends so it forces you to move a lot, just like in the game it is inspired by.

Hmmm.. try Parsec 47 by Kenta Cho... very fast pace.
