I don't quite get why everyone is up in arms. It's not the first time a right-to-left hori has been made, though there really havn't been any in recent years. It's not like you have to learn a completely new way to dodge bullets.
I also don't understand the Megadrive knockoff issue. Progear had this exact layout. Score, bombs, and lives at the top and a counter in the bottom left corner. If it followed the MD notion there would be a huge information box on the right side of the screen for the entire game.

Everything has been received particularly well by at least one group of people. And it seems that those groups of people are large enough to make all or most of these games profitable. The biggest problem has been Yagawa's games, which were unattractive to Ikeda's large following. But with Muchi Muchi Pork he seems to have found how to marry his love for heavy-rank systems to a style and feel which, at least on the surface, appeals to the average Cave fan.
... Heh, I'd welcome a Defender-stylee shmup from Cave. Though a true homage would ditch 8-way control and adopt the superior, original 2-way vert. stick, with 'reverse' and 'thrust': That would 'test' a few people!

Ikeda should've consulted with us first to see whether or not the mechanics will be approved. So far into the development and they still haven't figured out it's not a SHMUP lol.
I'm not too excited about this game. Well at least we know it's on the same SH3 hardware. We don't know if Ikeda is programming this one yet, do we? If he is, we know it won't be a maniac. He stated he would never make a hori maniac shooter again, after Progear.

Yes he probably won't take so many bullets there were in Progear no Arashi. We should see more bullets either.

I'll give you that aerial lolis are on the silly side, but it's not as if they've (yet) completely overrun the genre to the exclusion of all else, nor as if military-themed shooters (which I personally think have among the least attractive odds of ending up with really dreary presentation) usually make a whole lot of sense, either. And the phenomenon is far from new: Psikyo's deployment of alarming boobs in Sengoku Blade springs immediately to mind, and I'm sure examples could be produced going much further back. These games need to be eye-catching to do well out of passing trade, and if boobs'n'lolis achieve that, best of luck to them. You can always satisfy both fetishes with DDPDOJ - I still find it kind of jaw-dropping that they contrived a way of shoe-horning loli into that.
I don't know what everyone is complaining about. All you need is an inverted x-axis stick or controller setup and play in front of a mirror with the monitor behind you. Problem solved!

Slight OT: I can't see anything bad about bulletts behaviour in Progear, and I find they exploited the "gravity effect" on them in a very creative manner, to give players original bullett patterns never saw in other products of the same genere.
I can't understand why Ikeda didn't like his work on Progy...
However, Deathsmiles don't definitely look like a danmaku.
I'm very curious, to me it seem to be a very good product, but before we can see some action we can't give any judgement.
A Cave game that was just action and not really shooting would be a breath of fresh air, really
