Arashi (Tempest clone) for the Mac. One of two games bundled with the OS (Spectre Supreme being the other one).

Gyruss is my all time favorite game 
Also, add Juno Nemesis for the PC to the list. It was made by Mountain King Studios.
In addition, I'm creating a tube shooter (much like Gyruss) for the web using HTML5 and JavaScript technologies. Works with gamepads/joysticks in certain browsers, on iPhone and working on getting iCade support for iPad. You can actually try it out (no enemies yet) in its sandbox here:
