Just some things that popped into my head because I'm silly like that.
* Before the flashlight mod, how many Doom 3 players got eaten by a grue?
* Do you think Saw got his idea from going to a computer game store and seeing Phantasmagoria 2: A Puzzle of Flesh?
* Crazy Taxi driver vs. Mario Kart racer, who would win?
* The Japanese word for "the means" (as in "the means to do something") is shudan. The name of a lord during the Feudal era was Shogun. When you think about it, that makes SHODAN's name highly appropriate.
* Medal of Honor is a game with stealth elements, so why are all the end-of-stage ranks given to you exclusively based on kill count and how much health you have at the end of the level (making it waaaaay to easy to top-rank every mission)?

I wanted to answer the first one: DOOM III is actually surprisingly good with alarming you to taking damage. Even if I don't see something, I'll usually find it soon enough. The last time I played that game, I spent a lot of time (more than I wanted) replaying sections a few times in order to get through without dying and without constantly save/loading...not entirely successfully. Actually, I've never played the game with the ducttape mod. I won't say that the idea will stand the tests of time, but the atmosphere (or the attempts at it) worked better with the flashlight flip. Besides, my main use of the flashlight was to better admire the piles of guts in the corner.

This is more related to the movies, but since the Aliens franchise has games based on it...
The Xenomorphs' bodies are immune to their acidic blood, right? How hard can it be to replicate the structure of their exoskeleton and thus make acid-proof armor for the Colonial Marines? Or scavenge their dead bodies and make plates from the remains?
Final Fantasy VII. The Black Materia. Why not destroy it and thus rob Sephiroth of the only thing that could help him achieve his plan? What's Cloud and Co's excuse for keeping it around? The knowledge it contained? Which was only good to destroy planets? Which was something the planet didn't need? It's not like Materia is indestructible: The player has to infiltrate the Shinra rocket precisely for the purpose of saving the Huge Materia from being turned to dust at some point in the story.
(I just can't stand plot holes)
