Okay, so there's this great mystery about the NES shooter whose name is the title of this topic.
At the end of stages 1-6, there's a chance of rescuing a maiden. This changes the after-level cutscene and sometimes rewards you with health or extra lives.
Here's the thing: No one knows what the requirements are. So to the best of most player's knowledge, there's no way to ensure you saved one or not. Sometimes when I play I save them all, sometimes I only save the first one.. and I'm not sure what I'm doing differently each time.
I already checked at Gamefaqs--they didn't know. So I figured "the guys at Shmups are the experts. Do they know?"
So, do you?

For the Blue Dragon:
Stage 1: Always appears, leaves a message.
Stage 2: Firepower below three, restores life.
Stage 3: Appears at random (50% chance), 1up
Stage 4: Dragon has less than three heads, restores life.
Stage 5: Appears at random (50% chance), 1up
Stage 6: Dragon has less than three heads, restores life.
For the Gold Dragon:
Stage 1: Dragon has three heads, leaves a message
Stage 2: Appears at random (50% chance), 1up
Stage 3: Life is anything other than 3, restores life

I think you are the same person that emailed me years ago asking that same question. I believe that, because of you, I made a topic here years ago asking about this too 
No one seemed to know, until now, assuming what J_Taishu posted is correct.

The gold dragon route conditions don't seem to work. I tried the first 3 areas and the maidens showed up regardless of what I did.

But the info is very useful anyway. I'm now using a DVD Recorder to try and tape all the Maiden cutscenes just so I won't have to worry about it each time I play. Call it obsessive-compulsive.
It seems like the maiden text in Gold Dragon mode is exactly the same as in Blue Dragon mode. I wasn't aware they could appear unlit in Gold.
