The button mash sequences in Bayonetta aren't so bad, but it's also got horrible press X not to die stuff where you need extreme precision. If you press the wrong button or the right button too early too early as the colored 'halo' appears (or too late) it's a game over, and I find I mess them up all too often. They're some of the least forgiving quick time events I've seen.

Gimmicky effects to display low health, i.e. monochrome colour, lots of blurring, blood splatter all over the screen etc... The worst is when they do this even if there's a proper health meter on the screen. Doh. Hey, I know I've been injured, can you let me see what's going on so I can avoid taking more damage, thanks...

Five years ago I would have happily agreed with this, but lately devs have finally figured out how to use bloom effectively for creating transitions between different amount of light.
That's very true, it's improved greatly. But imo it's still more like someones imagination of what that effect looks like irl than what it actually looks like. Transitions between light and dark areas are a part of it that they've got pretty darn good at though.

Cutscenes, QTEs. Interactive cutscenes disguised as real play (see Assassin's Creed, where you do a lot of platforming that requires no skill and you can't fail). Unlockables.

Speaking of terrible effects, the depth-of-field effect. It may or may not look anything like what you'd see in real life, but that doesn't matter since people don't perceive the world as mostly blurred-out. Your eyes move all over to keep a sharp "mental picture" of what's around you at all times. If used in normal gameplay, the depth of field effect is not only unrealistic (because the plane of focus is generally tied to whatever the screen center or crosshair falls on) but it's a terrible disadvantage. As free as FPS mouse vision is, it's much slower than your eyes.

I'm playing The Run, and I can say with certainty that the thing that pisses me off the most are the "Takedown!" cutscenes. The camera spins around, you can't see where you're going, and I usually crash when the camera spins back around again.

EDIT: Oh, and slo-mo stuff that's supposed to be cool, but fails miserably because it just looks like everything is animating slower, which it is. And Max Payne has the advantage that he looks like he's tripping over his shoelaces every time. The Run has a good share of this, too. "Is it slo-mo, or am I just driving at 5mph all of a sudden?"
