The Deathsmiles games use 3 buttons by default, but are best played with a fourth button mapped to A + B for lock-on attacks and Power Up activation.

I have been trying every possible layout with deathsmiles NA on 360 with a good stick. I really don't think I would waste a button for lock. you just press two buttons. that was easy. I'm pretty sure the PCB does not have any way to control the familiar like on the 360. if it did though. I would map that to button 1 and bomb on button 4. I guess if the PCB supports lock then you can just wire it up for the forth button. on an arcade I probably would not use it even if it was there. if you press the two buttons to lock and you press one first you can choose left or right side for lock. I think this is a major advantage for players that use lock.

Yep, there's four button support from the get-go with the Cave SH-3 PCBs of DDP-DFK 1.5 & DDP DFK BL.

You'd just re-arrange the button placement to your liking on a typical control panel setup (assuming the panel itself is set up for a six button layout). Just swap out the QDs and arrange the wiring (no need to pop out the snap-in Sanwa/Seimitsu push buttons either). Easy as pie.

That's as lame as 3 x punch and 3 x kick buttons on Street Fighter games. <_<
