Pink Sweets PCB Problem SH3 (LED Status Lights question)

I have a Pink Sweets PCB (SH3 board) that seems to work maybe 1 in 20 tries.
When it first starts D6 LED is on and then goes off like normal but then 2 LED status lights come on (D1 and D4).
Any idea what those status lights mean?
Any help would be great!

Got a multimeter? Measure your +5v

also, do you have any other sh3 boards, and do they work ok?
