Psychonics OSCAR by Data East might be up your alley. Toki is one of my favorites as well. Heated Barrel is like a horizontal Gun Smoke. I own the PCB, only time I ever saw it for sale. Legend of Makai is kind of different as well.

Anyone that doesn't really know Motley Crue would probably have a WTF moment when they first fire up Crue Ball to see Alister Fiend pop up on the screen. Great game with a lot of laughs if you get the references regarding heavy metal.

Oh, and I know you don't want completely balls-out crazy but if you want a laugh you could always check out Muscle March. NIIIICE MUSSSSSCLE!

I'm pretty sure most of the Japanese Doujin games have never been heard of before in the west, so the list can be pretty huge!

I've yet to come across anything as weird as most early 90s PC Engine titles. Toilet Kids? Hani in the Sky? Cho Aniki? Seriously, there were some messed up games coming out during that era.

Messed up yes, but anyone who was seriously into Rom / ISO collecting in the late 90's and mid 2000s, would have heard and played all of those!

And as for the Cho Aniki series, well...
This game goes down in the annals of gaming history as the gayest shooting game ever!
Not many games can take that title. So enjoy.
Then fire up Dead or Alive 2, don't worry you'll be ok.

And then there is the more recent Choaniki: Sei Naru Protein Densetsu on the PS-2,
a Remake of Maisya / NCS's classic shooter from the PC-Engine, funny parody shooter, featuring two naked muscle men Adon and Samson as the main characters, and a bunch of muscle men as bosses.
The player's objective is to guide a legendary glob of protein to holy lands through groups of enemies that are aiming for the protein in order to get the ultimate physique.

I bought this game. Reason why I bought it: cuz the two guys on the cover told me to...
Do I regret it?
NOWAY..its a shmup!!!!

Another truly fantastic, and absolutely mind blowingly awesome (for it's time, especially) game is Out of This World (Another World in other places) for the SNES. Wow, when this came out, we were not ready for it, the cinematic approach they took with it was far ahead of it's time. The game play was simple, but adaptive making for a grand adventure. Still, pretty f'n weird. If you watch someone play this, it doesn't quite look like they are playing a game, more like they are watching it with you.

Worth also mentioning (running on same engine) Flashback by US Gold is also genius in this kind of way. The intro is still memorable to this day (showcasing latest edge tech for it's time) and the game remains as good as ever, though controls/mechanics get time used to and the game can be pretty unforgiving. It is not a good idea to try to beat it in one sitting (without abusing saves). It's best to tack on the game level by level since there is a password system. And it doesn't get any easier from now on. I can't say the say about Fade To Black, it's sequel which I TRULY disliked. The gameplay was stiff, it was full 3D instead of rotoscopic 2D. And that stuff

I'm surprised it took so long for Agony to get a mention. For more bird related strangeness check out Kolibri for 32x.

I've had alot of fun playing Mesopotamia over Magic Engine. It's quite a wonderful game where you control a mythological slinky birthed from an egg. It's sort of like an action puzzler. Check it out - http://www.youtube.com/watch?v=eFUQoHJ7zTI

Drum, are you including fighting games? If so then the PS2 port of Matrimelee (it's got a Japanese tag line I'm too lazy to look up) is one of the strangest. It is NOT a straight port of the Neo Geo game - the ROTD characters are gone due to legal reasons, as is Poochy, replaced by a fat kid with a bowl cut wearing a nappy, who now turns into Poochy as one of his special moves. And the end boss is now this African American guy who is actually a real life comedian who was popular in Japan at the time :? Instead of that little girl in the bishop's headdress. Forget his name. (Bobby Long)?

The fight stages are uniquely weird too, usually set at some musical event with the animated characters in the background and their movements synced with the stage tunes (all lyrical) which range from enka to metal.
There's a lovely duet in the karaoke bar, for example. To top it off it's published by the web portal designer Excite

Can't be the most known about game ever made either, as I collect all the 2d fighters for PS2 and I only really worked out what this was last year.

Even the vanilla Neo version can claim that it's the world's only marriage based fighting game, although given the bizarre nature of some of these niche Japan-only titles, I could be wrong.
