RQ: DDP 1 up on Stage 4?

RQ: DDP 1 up on Stage 4?

What do i have to do to get this 1 UP?
I have got it a few times but then when i really need it.... NOTHING??? :cry:

1. we have a topic for cave 1ups
2. I'm pretty sure that ddp doesn't have a 1up in stage 4 (also stage 4 is fairly easy as far as survival goes)
