NAME [INITIALS]: Your username, and then the initials you entered into scoreboard (in square brackets).
SCORE: Use some sort of punctuation (x,xxx,xxx).
LEVEL: Just the number.
ACCELERATION ON/OFF: Just the choice.
AUTOFIRE ON/OFF: Just the choice.
