PS2 Ibara booklet question

The little booklet that is mentioned when people talk about/sell Ibara, does that come inside the dvd case, or is it a separate item?

Am I right that this was shrink-wrapped to the PS2 case on the outside? Sometimes the book is attached, sometimes not. That might actually just be something certain retailers/distributors did of their own accord (I think that happened with the Cave Xbox 360 DLC cards; I'm sure the way my Deathsmiles II card came was different to some other peoples').

It was a pre-order bonus only (or at least until stock ran out). And yes, it was packaged separately on the outside of the sealed game in another resealable plastic wrap to keep them both together.
