Some of the information seems to be factually inaccurate, for example the 2nd loop requirements. Anyone have a wikipedia account and knowledge of the game?

The 2nd loop section is fixed, the requirements were correct, but it just didn't specify you only need one of the requirements. The rest looks fine...
