Wow, thanks for all the replies. I've taken a spin in almost all the games listed now. Some stand out:
Hi no Tori
Feels like the father of Undeadline and Elemental Master. I think this and Aleste Gaiden are the only walking shmups where you can jump. I'll probably appreciate the game more when I figure out how to open the damn door.
Ninja Emaki
This game is completely insane. In a good way.
Gun.Smoke
Simple & fun, a la 1942.
Extermination
I didn't remember this game when I made the topic, but I've actually played it before. They say people block out horribly traumatic experiences.
SWAT
Doesn't fit in with the theme, but it's really fun and creative. SEGA!
Psycho Chaser
The MSX games have better backgrounds than this. The MSX1 games.

This is really from the guys who made Out Zone? They must have hit the bottle hard.

Also in the vein of Out Zone is Gundhara, by Banpresto. Maybe arcade only.

I clicked on this thread thinking it'd be really lame. To my surprise, I stumbled upon a wonderful resource of games I've never heard of but sound really cool (all that Konami MSX stuff - I never knew!) and some oldies but goodies from my collection that've been neglected (Twinkle Tale & Undeadline, wow, it's been awhile since I've had a go at those).

Its not that bad. It is a difficult game because its one of those where if you die in Level 4 you are going to have a hard time coming back. One that immediately comes to mind is Heavy Barrel (Data East). Even though it's probably more of a top-down run and gun shooter, there are elevators and such that are also encountered in parts of the game.
It's probably very hard to find an actual arcade machine of this game nowadays (and the NES version was okay but was also significantly easier than the arcade version), but to me, it seems it would qualify as a shump with people, rather than planes or ships.

Isolated Warrior is an interesting ground based isometric shmup on the NES. Some of the later levels looks quite hectic:

What about Ninja Commando? You shoot in one direction only, like in a shmup, but the levels doesn't auto-scroll, which makes it feel more like a run'n gun. (Just noticed it was already mentioned)

Fray in Magical Adventure is another shmup where the character seems to (mostly) walk on the ground. I've seen pictures that show the heroine flying above ground too. The game was released for the Turbo-R(?) and PCE-CD.

The raft stage from the nes game 'The Adventures of Tom Saywer' is by definition a ground based vert. In the Japanese original it's the first stage too.

I prefer that ground based shmups take on some of the characteristics of horizontals myself though, like natural obstacles and that the enemies themselves become as much of a threat as their bullets.

As close as I've seen to Chaos Engine with forced scrolling, which means I might have to hunt that down.
Gatchaman: The Shooting is really, really, really, really easy on defaults and gets boring fast. Yes, even easier than Harmful Park or TFVI or Shienryu Explosion. On the defaults you can take a billion hits before dying...on the other difficulty mode you take one hit and it's Game Over. Yes, just one life. It still doesn't make the game any more interesting.

I found this eventually. Yeah, it's pretty easy. Kind of a Psikyo game for babies sort of feel to it.
