I would say that in ESP Ra.De. bullets don't have a small hitbox. In comparaison with DoDonPachi, with the blue bullets from 3rd boss, there is a big difference.
The hitboxes I showed come from the french forum. Hyryu marked a lot of them.
Just protect your neck. It's very odd to get the hang of, especially if you're used to any other Cave game where the hitbox is almost always in the geometric center of the character sprite.
I mainly play as Yusuke, and I seem to be hit in the forehead a lot. I figured the beret is deadly, but I guess it's just those blue bullets from the ground that are too fast for my eyes. I want pink bullets damn it!
It's fun to view this thread through the eyes of someone who doesn't play video games. Pictures of little kids with the base of their skulls targeted. Scary stuff.

Now that we have Esprade on Switch, we can turn on hitboxes. Turns out the hitboxes are actually in the middle of the character's back (instead of at the neck as believed here before):
When they say they hitbox is at the middle of the back they're referring to where to locate its center point based on the neutral animation when you're not moving around. It was thought at one point that the center was higher up in the neck area.
For my own purposes I just think of the visible hit boxes as an improvement that's a part of the game now. It's not that I found them excessively difficult ot keep track of, but it's a nice way to update the game with what's become a common feature (pioneered by Cave?) anyway.
