I just keep my DDP-DOJ PCB (with metal case and Cave protection seal still intact) with the cooling fan stock and no mods to it at all...
Keeping it cool as Cave intended it to be from the get-go "powered up"... ^_~
The cooling fan on such a metal-encased DDP-DOJ PCB isn't as loud as a factory installed CPS2 based one -- that generates such a very high pitched whine as it is when turned on...
PC Engine Fan X! ^_~

This board is perfectly safe to operate in the case with the fan disabled. The only component that might require any sort of cooling in extreme heat will be the Voltage Regulator, which already has a heatsink on it.
If you game in the Amazon or a dungeon then maybe you should consider keeping the fan enabled, but otherwise you will be fine. The PGM's hardware is nowhere near robust enough to require any sort of cooling.
