Fill me in on the last 5 years of STG
Out of touch. What is happening? Any new games. Which content creators are crushing games? Any developers come out to rival the Cave-like STG? Any updates on hardware or emulation?

Haha, wow, five years. Welcome back!~
I think, easiest summary is: honestly, never been better to be an STG fan, really!
So many good doujin STG being released all the time... so many reboots, remasters, re-releases, and so on. Even Steam!
For Cave-like, my more recent favorites are Gunvein, Like Dreamer & Blue Wish Desire! And if you played Blue Revolver... Double Action update was released.
You picked a good time to come back!
Every week, I see the STG groups I follow in Steam grow and grow, little by little. We are making many new fans as well!
Even for me... can consider me quite a "recent" newcomer haha. I will call myself new, because, took very long break, and in past mostly play "euro"-shmups.
I think, easiest summary is: honestly, never been better to be an STG fan, really!

On the Sega Mega Drive/Genesis console, two brand new stgs of Earthion and ZPF are worthy of your time for the 2025 timeline. The v3.3.0 of Earthion is the latest version to be released on Steam, Switch, PS4, PS5 & MD/SG.
Limited Run Games plans to release an official Sega Genesis boxed Earthion bundled set sometime in nearby 2026 -- a realistic MSRP of $49.99 - $69.99 seems quite possible (although pricing hasn't been officially announced/determined as of yet).
On Steam, the incredible "Devil Blade Reboot" (with seven years of development time to create it), it's quite impressive and runs flawlessly on a 1st-gen Steam Deck setup (from it's February of 2022 USA launch debut).

Between these three, I catch almost all new releases. I like Steam Curator a lot, because of mouse-over preview.
It lets you preview game screenshots to get quick general vibe of game and art style and sometimes even type of STG (verti, hori, etc)... so easier to play catch-up haha.

Well more like did not realize: Crimzon Clover World EXplosion was also released (on Steam, anyway) in the last 5 years @_@!~
One of the best STG's ever got a bit more polish and arranged!~
