Huge thanks to @viletim for helping me and providing the schematic for this, couldn't have done it without him. Posting it incase it helps anyone trying to achieve the same thing.

I finally have H & V control from the Extron 201RXI coming from the Mega Drive CSYNC! 8)

Quick note; I was having sporadic sync issues when testing through my breadboard, this chain is definitely picky on cable length, shorter the better.

The ISL59985 datasheet shows 0.1uF cap from Vdd to ground, this caused me issues and worked perfectly once removed. 56nF cap to ground from pin 6 CSET was needed as well.
