Are counterfeit copies of this Genesis/MD set floating around? If so are they hard to distinguish from authentic?

Not heard of any...

It's a well made product so I imagine any bootleg will just look/feel slightly cheap somewhere about it if someone did repro it... Got a suspect example?

Usual bootleg giveaway tends to be things like sharp corners on cart pcb connectors (should be rounded) / loose or low quality plastics on case, missing parts or poorer quality printing.

... I don't know if there were regional/store differences, but I wouldn't have assumed so...
... I'd assume bootleggers better off trying to bootleg originals more than re-releases so I *guess* you'd probably be fairly safe.
