Chaos Field for GameCube

Anybody know where to find it? Is it a Japan-only release?

Yup, it's Japan only. You can play it with a free loader disc. Just might wanna save your scores on a separate memory card to be safe (from what I have heard in some cases the GC free loader disc erases saves) You can order it here from NCSX.com, they're definitely a good dealer and reliable:
