When did games from different regions stop running at different frequencies (50 Hz/60 Hz region switch mods)?

For a wide audience and for a system's whole library only with the introduction of HD (PS3/360).
Prior to that ED (480p) capability always forced 60Hz at the same time (Dreamcast for example). For 15khz some games offered a 60Hz mode, but this wasn't universal by any means.

no. Just think of it: SD consoles still had to follow their countries TV standards. Breaking apart from that only became possible with HD.
