Not everyone successfully gets the head off. Back in the day you could lay your head across a traintrack, but the car/rope/immobile object combo is no doubt the most convenient modern method. You get to some strange places digging into that can of worms. This blog post has some pretty insane stories, most epically the man who built himself a guillotine and inscribed “This blade cut Arthur Charollais’ neck, October, 1900” on the blade.
