Shou's opinion (in 2008) was that there were Korean "hack PCBs," which suggests an unlicensed copy of the Mega Drive + game with some modifications. However the PCB photo there looks like a legit Data East board with labels and official Sega chips (the FC1004 is a Genesis sound chip), while the screenshot doesn't look like it's been modified with a coin counter. Safe to say *some* people have learned more about this one but it's still rather mysterious to me.

There is some good and bad news: The good news is that it's in MAME with some work done on the driver in Github as recently as March 14. The bad news is that the driver isn't working. I seem to recall the Mega Play / Mega Tech games took a long time to appear working in MAME; maybe this will or won't show up sooner than that. At the very least you can probably investigate the ROM data with your favorite Genesis data hacking utilities.
