Pretty much this:

I also realized while playing that the bullet appearance shows how it's aimed. The blinky blue bullets are always aimed, fire bullets are an aimed spread (either directly at the player, or shifted a bit) and the purple-black bomb things are both aimed and fixed (fucking NMK), and that's about it.
