Surprised nobody has mentioned the Gummi Ship stages from Kingdom Hearts 1 & 2. Though they're rail shooter instead of tradition shmup style.

The old side-scrolling Duke Nukem games for DOS were sorta shmup-ish when you found a vehicle, hopped in, and started flying it.

UltraSeven on the Super Famicom has a few similar bonus shooting stages, and one "for real" shooting stage where you take down a "boss"'s parts.
They are horizontal and break up the action from the VS. fighting decently.

Props to the guy who mentioned Xain'D SleenA! Xain!!!!!!!!
Wario Land: Shake has them, sort of a callback to the scrolling stages in Mario Land.
Shadow of the Beast has a shmup section - it is pretty dreadful.
Kid Icarus' final stage was a scrolling shooter.
Wizball has scrolling shooter bonus stages.
There is one in Wizkid: The Story of Wizball 2 that is sort of a throwback to the bonus stages of the first game. Also, the final 'boss fight' is an Asteroids clone. Love this game so much.
Super Empire Strikes back has a couple of horizontal scrolling segments - once when Luke is racing on a speeder bike towards his snow speeder, and again inside his snow speeder just after a free-roaming 3D section where you have to topple walkers.
Cybernator had at least one too.

There's a pretty dope shmup stage in Jade Empire (XBox). I really enjoyed it.

Most of the second area, and a few other interspersed levels in The Adventures of Batman & Robin (Genesis) were horizontal shmuppy in nature.
The second area (Two-Face's "flying over Gotham" stage) is pretty ridiculous if you're underpowered. You just keep slamming into bullets.
Captain America and The Avengers (Arcade/Genesis) had quite a few shmuppy bits in it as well.

Shooter/bullet hell elements in non-shooter games?
Should this go here? I've been pretty fond of this element in games as of late.

The red star actually has a full shmup stage (you have to destroy a giant tank)
And Beautiful Katamari has a great shmup stage you play when you clear the game. Its katamari mixed with Vert Shmup and is highly awesome!! Well done namco!!
