Where did the concept of 1cc as a game philosophy originate?

I think (Super) Monkey Ball did the super cool thing - to play extra stages, you mustn't fall even once before you get there. You can continue through extra stages, but the no-miss requirement up until that point makes them feel truly extra. (Not sure if they are accessible in the stage select mode, but I doubt it.)
