For the stage 4 part it's like this: you'll see a few purple orbs and one big red orb inside that big circle in the ground. Destroy all the purple orbs and then the red one...this will make the huge medals appear. If you are on your last life when you do this, a 1UP will appear as well.
As for score, it's really just a matter of generating as much medals as possible per level, especially on bosses.

Don't feel too bad about your score. Scoring is exponential, and tons of your score comes from speedkilling the stage 6 boss.
Like everyone said, it's all about medaling. You need to milk the bosses especially, and definitely don't break your chain by dying. That makes the game anal for scoring, but since you like DDP games, you're used to resetting when you lose your chain already.
Maybe you already know, but something else to keep in mind is that you should actually reflect the bullets back at enemies instead of just reflecting them anywhere. This way you get more medals of course. Point blanking is good. You should be pretty much reflect forcing non-stop too.

I got helplessly owned by the heart's first attack where it uses the red slow bullets. I reflect and move back to bottom... only to see an impenetratable wall of bullets closing in with no bombs in stock. Painful when you can see your death coming few seconds before you die and nothing you can do :(

I recorded .inp from the run but it syncs out at first stage ><. Damn it was so cool to dodge the first attack from the final boss (The plane) without bombs when I was just randomly moving around. Even more hilarious was that I managed to lose 2 lives to the next attack because I was so confused about being able to dodge the first one :P. Not so good score but I don't think I want to start milking bosses...
EDIT: Apparently the input desyncs totally randomly and it desyncs in different place each time. I managed to make it running without desync after a few tries and recorded the last 3 stages. Well, not like anyone cares but here is the Video
