The only difference is that AES carts are sometimes tweaked for being on a home console. For example, Metal Slug 3 AES has a 5 credit limit to artificially lengthen the game's lifespan for the home buyer. In terms of arcade perfect, the MVS carts are the arcade games, so you can do no better.
Cosmetically, MVS carts are pretty dull compared to the AES' kickin' clam shell cases. I like the ones with the buckle myself. But $1300 for a Metal Slug with a neat case vs. $60 for just the game is not a very hard choice to make, at least for me.

Wait a minute, either I'm blind or shocked. Neo Geo discussion and everything, and nobody has even mentioned Pulstar or Blazing Star? And we have mentioned Last Resort? Bizzaro world indeed. I think Pulstar and Blazing star are both excellent horis, and even the people who find them overrated would probably admit that they're pretty high profile games. Neo also has a few sort of crap horis - Zedblade and Andro Dunos come to mind.
Couple more suggestions:
- Sol Deace (Genesis/MD) aka Sol Feace on SCD - shockingly competent US Sega CD pack-in title. It's really quite solid!
- Sengoku Blade - Psikyo's horizontal take on the ancient Japanese myth theme they went with for the vertical scrolling Sengoku Ace. Not a bad game! There's also the not-so-good Sengoku Cannon on PSP.
Couple more suggestions:

EDIT: Sengoku Blade is called "Tengai" actually I already added it, how is that game? It seems so so to me, looks like Gambare Goemon shooting style....

If your into PC Dojin games, I should mention that the Air Rade series is excellent. I've only played Air Rade Air a little bit, but I love it. Angeraze is pretty good too. I'd recommend checking both out and adding them to your list.
Since you aren't looking for hori shmups on Genesis, PC Engine, or SNES, I won't bother mentioning any, but I've gotta tell you you're missing out on a whole lot of 16 bit goodness.

Wolf Fang/Rohga is great. It isn't considered a "real" shmup, same as Atomic Runner, but it autoscrolls.
Not too far away (and with traditional straight-forward aim) is Tengai, which I just played again for a bit. I like it a bit more than the prequel, Samurai Ace, but both are pretty good.

Actually, that is a difference in the systems bios, not in the cart. The ROM chips inside MVS and AES carts are identical. The option to play in Console mode or Arcade mode can be selected on either system if you install the Unibios. For anyone with a cart based Neo Geo I highly recommend installing a Unibios chip. Especially to MVS owners since the bios is socketed. The chip should only cost about $32 or so shipped from www.universebios.com

I might get Phalanx since it's out on GBA, but I don't feel like playing 16 bit games on a dusty snes. Plus you don't realise it, but snes games might look kinda slow, you know? Tengai's not much of a cute-em-up, really. If you want to try it it's available in MAME. Or just look around for some screenshots and reviews, they're out there.
As for Kolibri, IIRC it's got a weird setup, some stages are "traditional" scrolling shmup levels, while others are "limited roaming" (not sure of the correct term for them) levels more along the lines of Fantasy Zone.

Defender and Defender II- Genesis, SNES, PS1, Saturn, N64, PS2, XBox, GC, PSP, GBA, DC, NES and PC on various compilation packs and carts. The main one that got re-released over and over again was the Midway's Greatest Arcade Hits Vol 1. This thing was ported to just about every main console over the period of a few years. Forgotten Worlds- Genesis, PS2, XBox and PC Engine. The PS2 and XBox versions are on Capcom Classic Collection Vol 1, and are basically arcade perfect. The Genesis version is a toned down (but good) port, while the PC Engine version is on CD, is much closer to the arcade version, and has remixed music and new voice options.
