I second Planet Harriers. It's part of the reason I wanted a GameCube! Mario Kart GP seconded also, and R-Type Leo and Rave Racer. Though, Rage Racer is somewhat similar in tone.
OutZone could've been ported pretty well to the Genesis I think. It's a shame that never happened.
It's not a port from arcade, but I was really looking forward to that Genesis port of Vette before it got canned. Of course, that was before I learned that Vette was a load of crap...
Sega Strike Fighter? I wanted a port of it, but then I played it and I don't think we missed much. It would've been nice to see a lot of those later Naomi games on Dreamcast though, just for direct porting curiosity.
OK, I'd nominiate a console version of Speed Buggy, Super GT (duh!
), Zero Gunner 1 (just because I like 2), a 32x or Saturn version of OutRunners (you ported that to the GENESIS? WTF!), a console port of Hot Rod, and I think there was a console port of Off the Wall that never made it out the door.
I feel like there are a bunch of no-brainer games that should've be ported back in the day, but my memory's failing me.

Miscellaneous action games from the late 80s to mid-00s. I'm happy with most of the staple arcade genres' showings on console, but no Gundhara, Gamshara, Geostorm, Osman, Rolling Thunder 2 or Saigo no Nindou burns my ass slightly. RT2 and SnN's MD and PCE conversions are both pretty nice, at least.

I remember some kind of interest being expressed regarding IIDX Sirius (17) but it seems to have fizzled out. I eventually just lost interest and was lucky enough to live near a p-amuse connected machine running Lincle at the time, may it rest in piece.
Perhaps with Tricoro taking the series in a different direction it will be considered again, but I'm not really going to hold my breath.
Personally I've love to see a proper port of TGM, TGM-ACE is alright but whilst I lack a subscription to XBL I'm missing out on quite a few things, it feels like a shareware version. The version of TGM floating around for the DS was fantastic, something that faithful would be swell. But alas, the odds of that occurring aren't so favourable. (Or perhaps I missed a memo)

Oddly enough, all the local IIDX players I know kind of giggle at "casual hero". They don't really cater to the same crowd. Half Life 2: Survivor, because I think playing Half Life 2 with a scoring system would make the game even more awesome than it already is. My guess is that it wouldn't be too hard to port either, seeing as how it is still running the Source engine.

The PSX version isn't a port, it's a pixelly mess with a 3cm draw distance that's meant to resemble the arcade original, but doesn't really cut it. At all.
This 12 seconds of low-fi footage is all I can find of the arcade original (and despite the header, this definitely isn't the PSX version.)

Yep, that'd be the infamous Tengen TurboGrafx-16 prototype Turbo Chip cart version of Atari Games' Off The Wall arcade puzzler. Only one proto cart exists and it still hasn't been rom-dumped yet -- sure'd be nice to play it on the ol' PCE or TG-16 console setup indeed. Had to break down and get the original Jamma PCB of OTW just to properly play it -- you can play it either with all-digital based joysticks (via the 56-pin Jamma interface edge route) or the more proper rotary-knob spinner controller setup with up to three players simultaneously (via a multi-pin harness directly on the OTW PCB itself for players 1, 2 & 3 respectively). My local Funworks arcade joint in Modesto, CA had such an OTW Jamma conversion kit installed in a generic Dynamo cab with a larger control panel with Atari-branded three rotary spinners installed (to accomodate the third player easily) back in 1991. It was awesome to play it back in those days, especially with the ball approaching insane speeds as listed with appropiate speed descriptions to read at your leisure during a serious session of OTW. Plus, it'd save both current high scores & high score initals for posterity as well.

Ah yes, the ol' Sega Star Wars Arcade deluxe sit-down styled cab is still up & running at my local Funworks...takes arcade tokens to credit it up as the "gold choice" these days.
Not mention it has a Sega deluxe SD motion-based G-LOC cab as well for the classic arcade game purists like myself -- that's one you don't see anymore these days as it's a relic from a long ago era known as the "Silver Age of Arcades" lore. It certainly takes up some valuable real estate flooring space just to be properly set up (is one cab that still hasn't been properly shown at any of the past California Extreme shows either).
