The Cigarette Smoking Man in the X-Files *never* inhales. I mean, I get why, since he takes a drag in every single shot, but still. His fake inhaling through his nose is annoying. Trying to line up the crosshair with that little tin can, and move it upwards just on hitting it so you can walk out 4-5 hits. Hard to do consistently.

Getting stuck behind a State Trooper on the highway on my way to work in the morning.

Gimmicky levels that detract from the focus of the main game. Konami seems to have a problem with this. I'm thinking of Contra I, II, III here. The faux 3-D levels of part 1 are slow and annoying, and the overhead levels of III stupid and they make me kind of motion sick. I mind the overhead levels of II the least.

Ajax, another Konami game has some kind of Afterburner-ish mini levels too, which are short but still annoying. Metal Black has something similar.
