Having a guy with his eyes half open as if he's just woken up with a hang over isn't a role you're going to be playing in-game. What, no eye-lid control?
In-game engine != actual gameplay
Resident Evil 4 had plenty of cutscenes, complete with effects you didn't see during actual gameplay, but they were all rendered with the in-game engine. I don't think anybody is saying "wow, in the real game there'll be a button to shield your eyes." When people say this is in-game, they're referring to the engine rendering the graphics, and to me this looks like an advanced version of the RE4 engine on a new system. Considering what RE4 already looks like, and what Doom 3 and Half-Life 2 look like on current high-end PCs, I don't really see anything beyond the realm of possibility here for a new system.
Fake or not, I have no doubt we're going to be seeing games that look like this on the next systems.
Edit: Video up at IGN. Definitely looks real-time to me... Capcom's CGI is usually better.
