I've 1CC:d Toki, back in the day. It's a memorizer, and only the "roller coaster" ride at the end demands fast reflexes if I remember correctly. A much harder game is Kid Niki. Very cool though. Or Tiger Road. And btw, Surprise Attack is a real shit game.

I don't know if it's considered at all obscure (probably not), but Black Tiger is one of the best arcade action platformers ever made.

Wonder Boy III is kick ass, each level is divided into 2 sections; 1 half is platform and 2nd half is straight shooter.

Hey, it's not that hard to clear. I do it everytime I play it (I have the US Black Tiger PCB). Thing is, it takes you about 40 minutes to do that. Edit: More tips: Psycho-Nics Oscar, from DataEast. It has the Gradius power up system and cool looking bosses. Also, Chelnov Atomic Runner (autoscrolling platformer with nice weaponry where you change the characters direction by using a "turn" button.

Since somebody already said Spin Master I thought I'd at least mention Top Hunter. Has quite a lot of jump'n run elements in it and has excellent graphics.

Ninja-Kid II is awesome. It has several jumping techniques that you'll need to master, such as bouncing off of walls + swimming stages. And some of the best video game music ever.

Angry tree huggers beat up and shoot animal poachers in engrish laden fashion. One of my favorite pcbs.
Also for a beat em up so bad/strange it actually becomes awesome because of it, try Knuckle bash.

Those are both standard beat-em-ups though.
Even though it's not an arcade game, and it's bad, I would add my vote for Deliverance, which I have on Mac and didn't know it was Stormlord II. Oh god!! How bad is Stormlord then?

Trine looks like an even more boring version of Pandemonium.
Deliverance is a terrible game, but the artwork is hella cool. Every fantasy game should be required by law to have giant nude fairies in it.
As for Stormlord itself, pretty much every version I've played has been completely terrible (and Deliverance is both much better and totally diferent), but I think the game was originally developed for the Spectrum or something. I've not played that version, but I think it's supposed to be OK.

Deliverance apparently came out for the Commie too, but jesus...the Mac version is just so intensely bad I can't...can't...it can't be better than any other game. They probably had to build much of it from the ground up or something.

I haven't played Pandemonium. I would agree that Trine is rather boring, in terms of gameplay, from the demo (also buggy, aka lol @ walking through ceilings), but the characterization and GRAPHIX make it somewhat compelling for me. I'll definitely get it sometime, although that depends on the price. The game will (apparently) have multiplayer (maybe just split-screen co-op though), which was highest on the list of desired additional features for the game, although I have no idea what that could add to the game. i found this in taito memories 1 joukan (the very first one)... "Bonze Adventure" also known as "Jigoku meguri".. you control this buddhist monk and journey through what appears to be a path to hell... there's a whole slew of strange ghosts and demon monsters as enemies (youkai) and some interesting platform/stage designs. Your weapon is a projectile, throwing beads in an semi-arc fashion... it can easily be compared to ghost and goblin's/ghouls games, except for the oriental/buddhist themes... it is also equally, brutally hard.. (i've yet to 1cc stage 2)... it also got a PCE/TG16 port.. (also on wii virtual console)
