It's not a terrible game or anything, it just seems like an odd choice for the top 10 2D games ever. I haven't played it in ages, but what I remember most are those god-awful platform sections.
The plateforming portions are what make it really challenging, Supreme master up till the final boss would be a cake walk without those up till the final boss(BTW I 1lC it on SM recently). I look at that as hard but fair, unlike the first level of BattleToads in Battlemaniacs. Which is the easiest level but the most unfair. I don't mind dying when I'm not fast enough or precise enough, but weird collision gets me steamed.

I just remember playing DD2: TR with a friend relentlessly on the NES (until his Mum came in at 4am telling us to go to sleep.)
It was pretty inventive (kicking people out of that Helicopter door was great), and it got super challenging. The disappearing platform routine toward the end was a killer, but I just remember the difficulty spike was nutty when you had to fight silver versions of yourselves (I think?) and then all the bosses.
Fantastic 2 player game.

Key shooters on the Megadrive must include the seminal Thunderforce series, Aeroblasters and Ranger X, and to a lesser extent, Steel Empire, Gynoug and Hellfire.
