Inconsistency between 224p grid size and Genesis games

I've recently started using the 240p Test Suite on my Dreamcast to calibrate my PVM's geometry for 240p/480i and 224p video signals. I've noticed that if I calibrate the 224p grid to fill the screen, the image for Sonic 2 is misaligned in the top left area. Hopefully you guys could lend some advice as to why this is happening and whether or not its normal. Here's some more info about my setup in case it is relevant:
  • PVM 20M4U
  • All systems thru a gscartswitch
  • Dreamcast via TORO
  • Genesis Model 2
  • NES (NESRGB 2)

Another issue I'm experiencing is not being able to fill the screen with 240p/480i signals. On my PVM I use the regular scan for 224p systems and underscan for 240p/480i since my PVM doesn't support a bunch of different settings like how higher end PVM/BVMs do. I'm fine with the image not filling the entire screen, but many games have colored overscan areas like SMB/SMB2:
